RE: Elementary School Breakfast Survey
Date: March 14, 2018

Dear Parents:

State legislation requires the initiation of a school breakfast program, unless a demonstrated lack of need exists. If specific criteria is met which demonstrates a lack of need for a breakfast program, the District may request an exemption from the regulations.

Proposed Breakfast:
Choice of cereal, a bagel, a muffin, a roll, a fresh fruit, juice, and milk would be offered daily.
The charge per breakfast would be $1.50. The charge for those eligible for reduced priced meals would be $.25.

The following survey will enable us to determine if a need exists. To qualify as a valid survey, a response rate of 33% or more of the children in each school must be received; therefore, we are counting on your input!

Please complete this survey by as soon as possible.
