Regulation 3035: Records Management and Access
Requests for records shall be made in writing to the Records Access Officer, P.O. Box 21, Chappaqua, New York 10514 or by email to firstname.lastname@example.org. Such requests shall contain the name of the person making the request, his/her address or email address and a reasonable description of the records being requested.
